Q. Can I download patient forms from the website?
A. No. Patient forms are no longer available to download from the website and must be received and completed through APC’s text-message service.

Q. Why did I receive more than one forms text?
A. If more than one child is scheduled, you will receive a separate text for each child. Please open and complete every text individually so we receive the required forms for each child before the appointment.
